I don't remember the first one I saw at the movies, but I remember seeing "Hush, Hush Sweet Charlotte" on TV and freaking out when the severed head came bouncing down the stairs.
Amityville Horror - 1979. It might have freaked me out more if I had not already read the book. Since then no one living in that house has reported anything strange so the whole thing was probably just a hoax anyway.

"Alien" with Sigourney Weaver and Tom Skerrit, still one of my favorites.
